Spinal stenosis usually becomes symptomatic after the age of 50. It is caused by degeneration of the spine (with rare exceptions in patients born with a congenitally narrow canal). They may also show limited motion or reduced sensation. in neglected cases, it may be irreversible.<\/li><\/ol><p>Walking distance gradually decreases, and the longer nerve compression persists, the less recovery of walking ability is possible.<\/p><p>\u00a0<\/p><p><strong><b>Conservative treatment<\/b><\/strong>\u00a0is not curative, only <strong><b>symptomatic relief<\/b><\/strong>.<\/p><ul><li>Most cases of mild stenosis can be managed initially with <strong><b>painkillers and anti-inflammatories<\/b><\/strong>.<\/li><li>In rare cases of acute pain, <strong><b>epidural steroid injections <\/b><\/strong>or oral steroids may be used.<\/li><li>Activity limitation and wearing a <strong><b>lumbar brace <\/b><\/strong>can help reduce symptoms.<\/li><li><b><\/b><strong><b>Bed rest should be avoided<\/b><\/strong>, as prolonged immobility can lead to other complications.<\/li><li><b><\/b><strong><b>Physical therapy <\/b><\/strong>is useful, including heat therapy, electrical currents, ultrasound, and exercises to strengthen muscles, improve endurance, flexibility, and spinal mobility.<\/li><\/ul><p>\u00a0<\/p><h3><strong><b>Surgery \u2013 Simple decompression<\/b><\/strong><\/h3><p>The goal of surgery is to <strong><b>widen the spinal canal and decompress the nerves<\/b><\/strong>.<\/p><ul><li>The <strong><b>basic surgery <\/b><\/strong>is a <strong><b>laminectomy<\/b><\/strong>, removing the posterior part of the spinal canal (the lamina).<\/li><li>If the entire lamina does not need removal, only a portion can be removed (<strong><b>partial laminectomy<\/b><\/strong>).<\/li><li>Large herniated discs may also be removed to further decompress the canal.<\/li><li>The lateral foramina are also decompressed (<a href=\"https:\/\/www.hopkinsmedicine.org\/health\/treatment-tests-and-therapies\/foraminotomy\" target=\"_blank\" rel=\"noopener\"><strong><b>foraminotomy<\/b><\/strong><\/a>).<\/li><\/ul><p>\u00a0<\/p><h3><strong><b>Surgery \u2013 Decompression and spinal fusion<\/b><\/strong><\/h3><p><a href=\"\/en\/spinal-decompression-spinal-fusion\/\"><strong><b>Spinal fusion<\/b><\/strong><\/a>\u00a0is indicated only in specific cases:<\/p><ul><li><b><\/b><strong><b>Absolute indications: <\/b><\/strong><a href=\"\/en\/scoliosis\/\">scoliosis<\/a> greater than 20\u00b0 or slippage of one vertebra over another (<a href=\"\/en\/spondylolysis-spondylolisthesis\/\"><strong><b>spondylolisthesis<\/b><\/strong><\/a>).<\/li><li><b><\/b><strong><b>Relative indications: <\/b><\/strong>severe stenosis, significant disc disease, or notable back pain.<\/li><\/ul><p>During<a href=\"\/en\/spinal-decompression-spinal-fusion\/\"> fusion surgery,<\/a> after decompression, <strong><b>screws and rods<\/b><\/strong>\u00a0are used to stabilize the vertebrae. After surgery (or 2\u20133 days if spinal fusion was performed), patients can return home.<\/li><\/ul><p>\u00a0<\/p><p>At home:<\/p><ul><li><b><\/b><strong><b>Mobilization is encouraged<\/b><\/strong>, but rest is necessary until surgical stress subsides (a few days).<\/li><\/ul><p>\u00a0<\/p><p>Activity progression depends on the extent of surgery, previous activity level, and type of surgery:<\/p><ul><li><b><\/b><strong><b>Return to work or driving: <\/b><\/strong>~2\u20134 weeks<\/li><li><b><\/b><strong><b>Return to light exercise and walking: <\/b><\/strong>~1.5\u20133 months<\/li><li><b><\/b><strong><b>Return to contact sports (e.g., basketball): <\/b><\/strong>~3\u20136 months<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-313411cb
